Students may not enter the clinic without a signed pass. The clinic does not dispense medication of any kind, but does call the parent/guardian for students who may become ill and secures medical help (911) for a student who may require it in an emergency situation. If it becomes ABSOLUTELY NECESSARY for the student to take medication of any kind during school hours, that medication must be kept in the clinic and the student will be responsible for dispensing his/her own medication from the clinic. However, WRITTEN PARENTAL PERMISSION must be on file to do so. Emergency and health forms containing pertinent data are kept on each student and updated annually. Students failing to submit proper documentation will not be allowed to register.
State law requires all incoming 9th grade and transfer students to provide proper documentation of immunizations, and a current physical exam. (DCF forms #680 and #3040). All students entering a new school in the State of Florida must have a physical with an examination date of less than one year old from the start of school. The physical exam must be dated within one year of the student's first day of school and must be repeated annually if the student participates in a school sport.
