F.A.C.T.S. MANAGEMENT FORM (OPTION 3)
2008-2009
NEW APPLICANT
• Your student will be considered a “New Applicant” if your family was not enrolled in this program during the current 2007-2008 academic year.
• You will pay your tuition in monthly installments.
• There will be an automatic monthly deduction through your checking or savings account on your choice of the 5th or the 20th of the month.
• Total tuition and fees are normally divided by 10 payments beginning in July and ending in April.
• Balance can be paid at anytime with no penalty.
• The annual, non-refundable F.A.C.T.S. enrollment fee is $41.00 per applicant.
• There are no other fees or interest with the exception of a $25 non-sufficient fund fee, by F.A.C.T.S., if your payment does not clear.
• The annual fee will be automatically deducted from your checking account when the application is received by F.A.C.T.S.
• F.A.C.T.S requires the responsible party to sign an agreement giving them authorization to withdraw the payments from your account.
• The form requires the responsible party’s bank account number and clearly states the number of payments and withdrawal amount.
• You must make an appointment with the school bookkeeper, Ms. Clymer @ 561-314-2111 to come into the school Finance Office to complete the form. Forms will be provided at that time.
• Appointments can be made after March 8th and they conclude on April 30, 2008.
• It is important that you read the brochure carefully before you arrive.
